Exclusions Apply

  • Plan cannot be combined with insurance plans, discount plans or promotions

  • This plan is only honored at Stratland Dental. It cannot be used at any other dental or specialist office.

Limitations

  • Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
  • Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
  • All payments are due in full at the time services are rendered.
  • No refunds.

  • Benefits are provided for 12 months from the date the plan is purchased
  • No other discounts with this offer.
  • Membership is not for re-sale.
  • All membership fees are due at the time of registration and are non-refundable.
